Medical Records Policy
At Heartland your medical records are strictly confidential. No information will be released to insurance companies, attorneys, physicians or other persons without your written authorization. For your protection, we cannot accept authorization on the telephone. If you wish to have your medical records released, you may send a letter requesting your records, where you would like them sent, along with your full name, address, date of birth and a telephone number where you may be reached. We have forms you may fill out in the office and perhaps most convenient, you may click the link below and print the Release of Medical Records form.
Pursuant to Section 164.524 of Public Law 104-191, the Health Information Portability and Accountability Act of 1996 (commonly known as HIPPA), health care providers such as Heartland Women’s Health Care may charge a fee for copying and mailing medical records requested by a patient. In Missouri, the maximum charge allowed for copying is regulated by state statute and changes each year.
