On your first visit to the office each year, we will ask you to complete a registration form to update your personal demographic information as well as your insurance information.  Accuracy of this information is essential for the correct filing of claims on your behalf, communication to you from our providers and staff as well as account statements.

Please inform the receptionist upon check in at any visit if there has been a change in your health insurance, address, telephone numbers or emergency contacts.

One final reminder, please turn off any call blocking apparatus on your telephone if you are expecting a call from our office or from one of our providers.
