Welcome to Heartland Women’s Health Care.  Thank you for choosing us to provide your obstetric or gynecologic health care needs.  We hope you will see what makes us the preeminent OB/GYN private practice in the Northland and why our providers continue receiving awards year after year.  Please take a few minutes and explore our website.  We have developed it to provide basic information about our providers and our office for your use.  We also hope that you will find it educational.

When you or your primary care physician’s office calls to make an appointment, you will be greeted by a live operator.  No automated system here.  We believe it is important to begin your relationship with Heartland Women’s Health Care with a real person.

During that first call some very basic information will be asked from you.  Time permitting, usually four business days before your appointment, you will be mailed a welcome packet which will include several important pieces of documentation.  This will include a New Patient Registration Form and Personal History Form.  Please take a few minutes to complete these very important forms.  Doing so will help minimize your time spent at the office prior to seeing your provider.  You may select the option below to print the welcome packet directly to your printer if you have not received this by mail.

Should you not have the opportunity to complete this information, please plan to arrive 15 minutes prior to your appointment time so that the necessary information can be obtained prior to your scheduled appointment.

In addition to completed forms, please have available your insurance card showing policyholder’s name, policy number, and group number along with a photo ID.  This information will be photocopied and included in your medical records.
